CENTRAL VERMONT MEDICAL CENTER

The classes are open to any individual with diabetes and/or their significant others.
Program: Diabetic Outpatient Services
Clinic is open 8 hours each week for any individual in need of information regarding diabetes management or nutritional consultation.

Program: Diabetic Education Classes
This 10 hour series is held every other month and covers diabetes etiology, medication and insulin administration, nutritional management, glucose monitoring, foot care and more
Individual counseling sessions with a Registered Dietitian and a registered Nurse is available with this program.

Program: Central Vermont Diabetic Support Group
The group meets monthly on the first Wednesday evening in a Conference Room from 7:00 PM to 8:30 PM. The program includes guest speakers, group discussions, and various activities relating to improving knowledge and self-management or diabetes

GIFFORD MEDICAL CENTER

Program: Diabetes Outpatient Clinic
Clinic is open 8 hours every other week for any patient in  need of diabetic education, dietary consultation, blood glucose monitoring, insulin administration, or basic diabetes knowledge.

Program: Diabetic Support Group
The group meets monthly from September through May at Gifford Medical Center from 7:00 PM to 8:00 PM. Format varies from group discussions, guests speakers, films.

DIABETES CENTER OF THE LAMOILLE VALLEY

Program: Introduction to Diabetes Management
Individual instruction on basic survival skills for the newly diagnosed diabetic and family members.

Program: Diabetes and You: A Self-Management Education Series For People With Diabetes And Their Families
This group education program consists of five 2 hour sessions preparing people to manage their diabetes. This program is appropriate for people with Type 1, Type 2 or Gestational diabetes. Faculty includes a Nurse Practitioner who is a Certified Diabetes Educator, a Registered Dietitian, and a Behavioral Therapist. Patients may enter the program through self referral or by referral of their medical provider.
Participants receive a comprehensive assessment prior to participation, and are invited to three follow-up sessions in the year following participation.

Program: Individual Outpatient Diabetes Education
Covers the content of the group education program when group education is not appropriate. Available by appointment.

Program: Insulin Start and Adjustment
Individual instruction is offered for the initiation of insulin therapy. In addition, frequent contact is available with the Nurse Practitioner/Certified Diabetes Educator to adjust insulin doses to achieve tight control.

Program: Individual Instruction Program: Diabetes: The Basics of Self Care (Group Class and Individual Sessions)
RRMC offers an A.D.A. recognized comprehensive diabetes clinic, which includes one on one sessions and a group class, which meets weekly for 6 weeks, each class lasts 2 hours. The course covers: defining diabetes, nutrition, medications, monitoring, exercise, stress reduction, acute and chronic complication

SOUTHWESTERN VERMONT MEDICAL CENTER

Program: Diabetes Education
Program Individual instruction, insulin pump therapy, group classes. Individual and group instruction for patients, their families and significant others to teach self-management of diabetes. Group classes are offered quarterly, as well as individual sessions by appointment. Outpatient diabetes program has been recognized by the American Diabetes Association.

Type 2 Evening support group: Meets last Wednesday of every month from 7 to 8:30 PM. Coordinated by Pat Carpenter, RN, CDE, CPT.

Type 2 Daytime support group: Meets every Tuesday from 10:00 to 11:30 am and is coordinated by the patients. A diabetes educator is available.

Type 1 / Insulin pump support group: Meets third Thursday of each month from 6:30 to 8:00 PM. Coordinated by Lynn Hurley, RN, CDE, CPT

VERMONT REGIONAL DIABETES CENTER / FLETCHER ALLEN HEALTH CARE

Program: Living Well with Diabetes (Group Class)
A basic diabetes education course designed for people who want a better understanding of diabetes and how to self-manage better. Support persons are encouraged to attend.

Program: Diabetes Self Care Management Training (Individual instruction)
Individualized instruction and training in diabetes self care skills. Can be adapted for patients with newly diagnosed or long term diabetes who want to improve their control. Assessment and content covers all 15 content areas required by ADA recognition. Includes help with technical training, behavior modification, problem solving and dose adjustment if appropriate.

Program: Insulin Pump Therapy Program (Individual instruction)
A comprehensive program for individuals interested in insulin pump therapy. Candidates are assessed by a multidisciplinary team. Program includes complete training in all aspects of pump therapy, as well as follow up.

Program: Insulin Pump Support Group
A support group for persons on insulin pump therapy, as well as their support persons. Individuals considering pump therapy are encouraged to attend. Discussions include guest speakers, sharing experiences and networking.

Program: Diabetes Essential Skills Training
A 4 hour program for individuals with diabetes and support persons in need of  individualized assessment and education regarding survival skills. Designed for newly diagnosed individuals or those beginning insulin therapy. Is also an excellent update for those with outdated practices. Taught by an endocrinologist, RD CDE and RN CDE. Meters can be loaned and insulin supplies will be provided.
