Programs of the Diabetes Center of Lamoille Valley



 
 
 

Basic Survival Skills Program: for newly diagnosed diabetics and people with poorly controlled diabetes (HgbA1C > 8.0%). Individual education sessions with the Registered Nurse and the Registered Dietitian are directed at helping the person with diabetes to begin taking the steps needed to control their disease. Visits are usually scheduled for three hours over two sessions. Family support people are engouraged to attend.

Group Diabetes Self-Management Education Program: a structured group self-management education programs that cover the 15 content areas identified by the American Diabetes Association as essential for such programs. The program is delivered in five two-hour sessions to people with diabetes and their family/support persons. Follow-up sessions are held at three month intervals for the next year to help participants maintain their commitment to self management while refining and revising goals. Programs have been held in Hardwick, Morrisville, Cambridge and Johnson. We will bring programs to your group of five or more.

Insulin Initiation Program: to assist the person with diabetes in safely initiating the use of insulin to manage their disease. Visits are usually scheduled for two hours over two sessions.

Insulin Pump Program:  Comprehensive training program for those seeking the flexibility and tight glucose control possible through the use of an Insulin Pump.  Initial training and ongoing follow up provided by a Certified Insulin Pump Trainer.

Medication Adjustment Program: The Nurse Practitioner is uniquely prepared to assist the person with diabetes in adjusting their diabetes medications to achieve optimal control. This is done in collaboration with the Primary Care Provider. Most of the adjustment can be accomplished over the telephone, minimizing the visits necessary for this service.

Ongoing Individual Education and Support Program to assist patients in achieving and maintaining good glycemic control (HgbA1C <7%).
